What to Submit with this Application Form

1. Appendix M
2. An approval letter from the school district research department or principal of the school in which you intend to do research
3. Recruitment scripts, consent forms (no waiver of consent is permitted for exempt category 1), any surveys or interview guides (where applicable)

Reminders for Exemption Category #1 Research:

1. All the educational practices you hope to employ must be accepted, standard educational practices.
2. If all students will be participating in the educational practice, a consent form to be signed by the student (and parent if under 18) which states that they allow you to look at the results of their participation in the activity for the purposes of research.
3. If all students will not be participating in the educational practice, you must explain what the students who will not be participating will be doing instead. It is important that you design your research in a way that does not penalize students who will not be participating.
