Visitor Certification Instructions

For visitors to the University of Minnesota who will be handling animals, IACUC requires that they assure the committee and the University that they have met their own institution's federal training requirements.

Visitors should follow these steps:

1. Download the Visitor Certification Assurance Form.
2. Complete the form and mail to IACUC (faxes are also acceptable to begin animal work, but a signed form delivered or mailed to IACUC is expected).
3. Upon receipt and review, visitors will be contacted that they are approved.
