Use these forms to request approval for the use of animals for the purposes described.

Forms Available:

All forms are in Microsoft Word Format.

 

Download Instructions:

  1. Right-click on the form links below and select "Save Target as..." or "Save Link Target as..."
  2. A dialog box will appear and you will be prompted to save the file to your computer. Choose a location to save the file.
  3. Open the file from your computer using Microsoft Word or another compatible word processing program and complete the application form.

When to use this form:

  • For both lab and agricultural animal usage.
  • To apply for any biomedical or agricultural research (including teaching and classroom activities involving animals).

Download formBreeding Protocol

When to use this form:

  • This form covers only the breeding of animals and does not cover the use of animals for experimentation. A separate animal usage form must be submitted to cover the experimental use of bred animals.
  • If you will only be breeding animals, use this form.
  • If you will be breeding and experimenting with bred animals, complete only the Animal Usage Form and list your animals as being bred in-house.
  • If you will be obtaining animals from a group on campus that breeds animals, complete only the Animal Usage Form and list your animals as being transferred from their breeding protocol(s).

Download formAgricultural Husbandry Standard Operating Procedures(SOP)

When to use this form:

  • Agricultural animals being maintained for breeding, observation or display purposes only.
  • No manipulations will be performed on the animals or their environment. (Animals may be transferred, with IACUC permission, to experimental or teaching protocols and back again.)

Agricultural Animals: livestock or poultry used for activities related to food or fiber production, husbandry methods or health care of the target species.

When to use this form:

  • This form should be used ONLY for animals that are for display purposes. If animals are used for ANY experimental or teaching purposes the regular animal usage form or teaching animal usage form must be used.

When to use this form:

  • This form should be used ONLY for animals that are used solely in teaching purposes. If animals are used for experimental purposes the regular animal usage form must be used.


Download formWildlife Protocol

When to use this form:

  • Use this form when studying or obtaining wildlife from their natural environment. If this is biomedical type research please use the regular animal usage form.

When to use this form:

  • For custom antibodies that you produce or for which you direct production.
  • This form is not required if you will be purchasing already prepared antibodies "off the shelf".

  • Use this form to request an additional title. Also complete an Appendix O if receiving additional external funding..

  • Use this form to request personnel changes to IACUC-approved protocols. All personnel added to studies are required to have IACUC training certification.
