How to Submit Forms

Submissions should include:

  1. The original application form and appendices, plus 12 copies
  2. Documentation of any required approvals or applications for approval from other committees and from cooperating agencies
  3. All supporting materials and documents, including protocol, descriptions of any investigational drugs or devices, and all necessary Appendices as directed in the application
  4. If receiving external funding for animal research, regulations require 1 copy of the associated grant for review and assurance to the IACUC that the animal work for which approval is being sought exactly corresponds to that indicated in the funding grant.
